- Management expects Medicaid margins to improve in 2027 after calling 2026 the trough year.
- Medicare Advantage margins remain below the company's long-term target range.
- Investors will look to the January 2027 report for updated assumptions and the next earnings outlook.
Elevance Health faces a sharper investor test after a strong share-price run: the company's improved 2026 outlook must now be matched by better insurance margins. In an analysis published Thursday, November 5, Trefis on Yahoo Finance highlighted medical-cost sensitivity, Medicaid recovery and the company's 2027 earnings plan.
Elevance lifted its 2026 earnings outlook after reporting third-quarter results on October 21. Management expects adjusted earnings of at least 29 dollars per share for 2026 and has set a baseline of at least 26.75 dollars for measuring 2027 growth. The company is targeting at least 12 percent adjusted earnings-per-share growth next year from that baseline.
Margins Remain The Main Test
The analysis points to the benefit expense ratio as a central operating indicator. The ratio stood at 90.6 percent in the third quarter, an improvement of 70 basis points from the prior year. Because medical claims absorb most premium revenue, even modest cost pressure can have a material effect on the insurer's remaining margin.
Elevance expects a full-year operating margin of at least 2 percent in Medicare Advantage, below its long-term target range of 3 percent to 5 percent. Management also described 2026 as the trough for Medicaid margins and expects improvement in 2027, with a longer-term target range of 2 percent to 4 percent. The pace of recovery depends on reimbursement rates catching up with the cost of care, according to management's October 21 comments cited in the analysis.
The third quarter also included a net non-operating benefit of 1.85 dollars per share, mainly from valuation adjustments within investment income. That item makes the distinction between reported earnings and the operating earnings baseline important for investors assessing the durability of the outlook.
