- Exchange coverage losses continue to shift patients toward uninsured care.
- Lower Medicaid supplemental payments are expected to weigh on next year's earnings.
- HCA plans to provide fuller 2027 guidance with its fourth-quarter results.
HCA Healthcare is carrying solid patient demand into 2027, but reimbursement pressure is clouding the earnings outlook. On its October 27, 2026, third-quarter earnings call, management said next year's earnings should be generally consistent with 2026 despite projected volume growth.
The company's preliminary view was outlined in the HCA Healthcare Q3 2026 earnings call transcript. Chief Executive Sam Hazen said demand remained strong and that volume could grow 2%-3% in 2027, while exchange-related coverage losses should have a smaller but still meaningful effect on payer mix.
Coverage losses remain a key risk
Chief Financial Officer Mike Marks said patients losing exchange coverage continued to move almost one-for-one into uninsured care. The shift reduced adjusted EBITDA by approximately USD 400 million in the third quarter, while uninsured patients accounted for 10.5% of equivalent admissions, up from 8.5% a year earlier.
HCA also expects USD 550 million to USD 650 million less in Medicaid supplemental-payment net benefits in 2027 than in 2026. Management attributed the decline mainly to state approvals received in 2026 for services provided in earlier periods. Cost initiatives and operating leverage from higher volumes are intended to offset part of the pressure.
January guidance becomes the next catalyst
HCA plans to provide more detail with its fourth-quarter results in January. Management's early view assumes that the most severe exchange-related effects will ease, that there will be no material policy changes beyond HR 1 and that strategic initiatives will create additional value.
