HCA Healthcare reported its third-quarter results on October 27, 2026, for the period ended September 30. In the HCA Healthcare release, revenue rose 3.6 percent year over year to USD 19.849 billion, while net income attributable to HCA Healthcare declined 3.9 percent to USD 1.579 billion. Adjusted diluted earnings per share increased 3.7 percent to USD 7.22, and adjusted EBITDA rose 1.2 percent to USD 3.918 billion.
Volume growth meets payer pressure
Same-facility equivalent admissions increased 3.0 percent and emergency-room visits rose 5.1 percent from the prior-year quarter. At the same time, inpatient surgeries declined 1.4 percent and outpatient surgeries fell 3.7 percent, indicating a mixed service-volume picture with stronger demand for emergency care than for elective procedures.
HCA said a shift toward uninsured patients, primarily involving people who lost health-insurance exchange coverage, had an unfavorable impact of approximately USD 400 million on income before income taxes in the quarter. The company also reported USD 4.461 billion in operating cash flow and total debt of USD 50.493 billion at September 30.
Guidance remains in the upper half
Management said it expects full-year 2026 operating results to land in the upper half of its existing guidance ranges. Those ranges include revenue of USD 77.0 billion to USD 79.5 billion and adjusted EBITDA of USD 15.4 billion to USD 16.1 billion. The outlook therefore depends on continued volume growth and operating improvements offsetting payer-mix pressure.
