Nearly 59 percent of German employees say their workload has increased, and almost half of HR managers report climbing sick?leave rates. The main drivers: persistent time pressure, unclear task assignments, and workplace conflicts. That is the finding of a survey by Civey and the HR software company Factorial.

The mental?health burden is attracting new technology solutions. The British HealthTech firm JAAQ has secured €15 million in funding to expand its platform, which combines artificial intelligence with clinically verified content. The company plans to bring the service to more countries, including Germany.

But the business case for prevention is already clear. The Aalen University of Applied Sciences calculates that every euro spent on structured return?to?work programs—known in Germany as Betriebliches Eingliederungsmanagement (BEM)—generates €4.80 in savings. A midsized company with around 500 employees pays between €15,000 and €25,000 for a well?designed BEM process. The resulting drop in absenteeism delivers savings in the higher five?figure range.

Broader workplace health management (Betriebliches Gesundheitsmanagement, BGM) also pays off. According to the BKK umbrella association, each euro invested in BGM yields an average return of €2.70. The cost of doing nothing is stark: the Federal Institute for Vocational Education and Training puts the average expense of a single apprenticeship dropout at €6,478.

Heat Waves Add to the Bill

Extreme weather is compounding the problem. Researchers at the Potsdam Institute for Climate Impact Research, using AOK health?insurance data, found that sick?leave notifications rise 3.5 percent on days when the temperature tops 30 degrees Celsius. After a seven?day heat wave, the increase jumps to 10.8 percent. A three?day hot spell causes roughly €32 million in extra wage?continuation costs. Construction, manufacturing, and transport are the hardest?hit sectors.

State and Insurer Support Available

Companies can tap several public and statutory programmes. The German Pension Insurance (Deutsche Rentenversicherung) in Baden?Württemberg runs a corporate?service unit that advises on prevention initiatives such as “RV Fit”, medical rehabilitation, and phased reintegration. The aim is to cut absenteeism and preserve employees’ ability to work until retirement.

Statutory health insurers are also required by law—§ 20b of the Social Code (SGB V)—to finance workplace health promotion. They cover certified courses in exercise, nutrition, stress management, and addiction prevention. Small businesses in particular can join cross?company programmes that may be fully funded.

Insurer Surplus Masks Looming Deficit

In the first quarter of 2026, Germany’s statutory health funds posted a combined surplus of €773 million. The AOK accounted for €206 million of that, while the company?based BKK funds contributed €300 million. The surplus stems from an increase in the average supplementary contribution rate. Yet experts warn that the picture will reverse sharply: a deficit of over €15 billion is expected for 2027.

One of the most concrete examples of successful workplace health management comes from the Saarpfalz district, which received a €10,000 award for its model return?to?work programme. District Administrator Frank John highlighted such efforts as essential for retaining skilled workers. The Saarbahn transit company, the St. Wendel district savings bank, and Hörmann KG Eckelhausen were also recognized. The prize is part of a statewide push to embed health strategies in corporate culture.
