A Swedish pension fund has placed an unusually large bet on Intuitive Surgical at a moment when the stock is plumbing fresh lows and a dozen sell-side analysts are slashing their price targets. Andra AP fonden boosted its holding by 139.7% during the first quarter of 2026, accumulating 50,050 shares worth roughly $23.1 million. The move comes as the da Vinci robot maker's shares hover around €310, down more than 36% year-to-date and just 3.8% above a 52-week trough of €299.60.

The fund's conviction stands in stark contrast to the message from the C-suite. Senior Vice President Iman Jeddi sold 5,625 shares in early June at $420.55 apiece, and Executive Vice President Gary Loeb offloaded 400 shares a week later at $424.14 — both transactions executed well above the current market price. The insider exits undercut what would otherwise look like a clear institutional buy signal.

Intuitive Surgical's latest quarterly numbers, released for the second quarter, would typically justify optimism. Revenue climbed 19% to $2.89 billion, beating consensus by 2.5%, while earnings per share jumped 28% to $2.80 — 12% above analyst estimates. The instrument and accessories line rose 18% to $1.73 billion, systems revenue advanced 19% to $685 million on 468 da Vinci placements versus an expected 444, and the services business grew 21% to $472 million. Yet the market dumped the stock in its steepest one-day decline in years.

The culprit was not the quarter itself but the outlook. Management left its full-year procedure growth forecast unchanged, resisting calls to raise it. In the US, da Vinci procedure growth decelerated to roughly 12%, the weakest pace since 2021. Piper-Sandler analyst Adam Maeder pegged the sequential deceleration at around 200 basis points, noting it missed Wall Street expectations. Evercore's Vijay Kumar called the US procedure softness the most pronounced in three years, rekindling debate about broader med-tech demand.

Should investors sell immediately? Or is it worth buying Intuitive Surgical?

The analyst community responded with a wave of price-target reductions, though almost no one downgraded the stock. Bernstein SocGen Group cut its target from $750 to $685 while actually raising its 2027 EPS estimate by 4% to $12.71 — a telling sign that the compression is about valuation multiples, not earnings power. Other cuts included JPMorgan ($550 to $450), Wells Fargo ($654 to $487), Stifel ($670 to $550), Citigroup ($590 to $500), RBC Capital ($600 to $575), Leerink Partners ($573 to $454), TD Cowen ($520 to $485), Mizuho ($500 to $400), Baird ($525 to $500), BTIG ($512 to $469), and Evercore ISI ($430 to $375). Leerink's Mike Kratky described the sell-off as "increasingly overdone," though he acknowledged that regulatory shifts in insurance reimbursement add uncertainty to US procedure growth.

The technical picture reinforces the bearish mood. The stock trades roughly 13% below its 50-day moving average of €358.26 and nearly 26% below the 200-day average of €418.94. The 14-day relative strength index sits at 34.1, flirting with oversold territory but without any confirmation of a reversal. Annualized 30-day volatility has jumped to about 54%, reflecting the turbulence since the earnings release. From a January record of €516.50, the shares have shed almost 40%.

The Q1 results, which preceded the fund's stake increase, were equally strong. Intuitive Surgical reported EPS of $2.50 against a $2.08 consensus and revenue of $2.77 billion, up 23% year-over-year. That quarter failed to arrest the slide, and the subsequent Q2 beat has done little better.

Intuitive Surgical at a turning point? This analysis reveals what investors need to know now.

Despite the gloom, the Street's average price target stands at $523.46 with a "Moderate Buy" consensus — a far cry from today's levels. Andra AP fonden's bet will succeed only if Intuitive Surgical's operational momentum eventually overcomes the market's growing doubts about its premium valuation and the trajectory of US procedure growth. Until then, the stock remains caught between robust fundamentals and a chart that has not found its floor.
