AOK Bavaria Spends Record €41.8 Million on Prevention While Sick Leave Persists Across Germany

Germany’s statutory health insurance system is grappling with a contradiction: prevention spending by its largest regional fund has hit an all-time high, yet the sector faces a looming budget squeeze. The AOK Bavaria health fund reached roughly 1.35 million people with its preventive measures last year — a 20 percent jump from 2024. Investment climbed to €41.8 million.
About 500,000 employees across more than 2,600 companies took part in workplace health initiatives. One-third of the spending went to nursing homes. The fund's "Move Days" events attracted over 6,500 participants from more than 100 businesses. In small and medium-sized enterprises, engagement ranged from 60 to 95 percent. For half of those attending, it was the first time they actively addressed mental health.
Those prevention efforts unfold against a backdrop of stubbornly high sick leave. In Saxony-Anhalt, AOK-insured workers were absent an average of 28.4 days in 2024, yielding a 7.7 percent absence rate. Respiratory infections and musculoskeletal disorders were the leading causes, while mental health conditions produced the longest individual absences — nearly 27 days per case. The variation across sectors is steep: road maintenance workers record the highest sick leave, university staff the lowest.

Despite the prevention boom, the statutory insurance system is under financial strain. The Bundestag is expected to vote on a savings package around July 10, 2026. Expenses in the first quarter of 2026 grew far faster than revenue, and the GKV-Spitzenverband — the umbrella body for statutory health insurers — is pushing for further cuts.
