Nearly every second German hospital is in the red, and the outlook is even grimmer. According to the 22nd Hospital Rating Report, released Thursday, around 50 percent of all German clinics posted losses in 2024, with the average annual result sitting at minus 0.6 percent of revenue.

The situation is set to deteriorate further. Analysts warn that under the planned GKV Contribution Rate Stabilization Act, the share of loss-making hospitals could climb to 60 percent by 2027. By 2030, as many as 70 percent of facilities may be unprofitable, jeopardizing roughly 400 locations. To stay afloat, non-nursing personnel costs would need to drop by an estimated 8 to 9 percent.

One flashpoint is the Schön-Klinik in Vogtareuth, where management temporarily shut down Station K on June 15. The clinic insists the closure is a temporary adjustment, with plans to reopen the ward on October 1, and stresses that no jobs have been cut. But the works council challenged the move, accusing the administration of bypassing its co-determination rights. On Friday, the Rosenheim Labor Court rejected the council’s emergency petition, siding with the employer.

The ruling exemplifies how economic pressure and labor rights are colliding across Germany’s hospital landscape, and the conflict is far from isolated.

On Thursday, employees of the Lausitz Klinik in Forst took to the streets. They protested against a cap on the nursing budget and the lack of state reimbursement for pay-raise costs. Their central demand: a rescue shield for the entire healthcare system.

Meanwhile, disputes over works council elections are adding fuel to the fire. The management of the Seniorenheime Dortmund group challenged the validity of its employees’ recent election – a move sharply criticized by the Working Group for Employee Affairs. At Frankfurt Airport, shredded ballots have complicated the debate surrounding Fraport’s works council ballot.

Some local authorities are scrambling to secure basic care. The Wartburg district, for instance, will acquire shares in the St. Georg Klinikum Eisenach from July 1, backed by new strategy papers designed to keep the site viable.

Whether these patchwork efforts can reverse the trend remains uncertain. The fight over hospital survival, legal co-determination, and public funding is only expected to intensify. The Vogtareuth case, say observers, is just a preview of what lies ahead.
