Intuitive Surgical delivered a record-setting second quarter, shipping more da Vinci systems than analysts had expected and posting a 19% revenue jump. So why is the stock trading just a hair above its 52-week low, down more than a third since January? The answer lies in an uncomfortable reality for growth investors: the company’s most important metric — US procedure volume — is slowing faster than the market anticipated.

The robotic surgery specialist placed 468 da Vinci systems in the three months ended June, comfortably ahead of the 444 consensus. More than half of those, 246 units, were the latest-generation da Vinci 5, underscoring the speed of the platform’s market adoption. Revenue hit $2.89 billion, roughly 2.5% above analyst estimates, with instrument and accessories sales climbing 18% to $1.73 billion, system revenue rising 19% to $685 million, and services adding $472 million, up 21%. Adjusted earnings per share came in at $2.80, also ahead of forecasts.

Yet the stock fell more than 10% in after-hours trading on July 16 and has continued to drift lower, touching €299.60 on Monday — a fresh 52-week trough. The trigger was a single number buried in the earnings release: US procedure growth decelerated to 11.5% from 13.5% in the first quarter, missing the 13.2% consensus by 170 basis points.

Management pointed to two headwinds: a decline in insurance coverage tied to changes in the Affordable Care Act, which is limiting patient access, and the law of large numbers making it harder to sustain double-digit growth on an ever-expanding installed base. Neither explanation mollified Wall Street.

Should investors sell immediately? Or is it worth buying Intuitive Surgical?

What really frustrated investors was the guidance. Intuitive Surgical merely reaffirmed the midpoint of its full-year global da Vinci procedure growth range of 13.5% to 15.5%. After a Q2 that topped expectations, the market had expected an upward revision. Sticking with the same forecast implicitly signals a slower second half, a message that clashes with a stock still carrying a premium valuation.

Analyst reactions have been swift and split. Bernstein SocGen Group cut its price target from $750 to $685 but maintained an "Outperform" rating, even raising its 2027 earnings per share estimate 4% to $12.71. Citi lowered its target to $500, keeping a "Buy." JPMorgan trimmed to $450. Mizuho went further, slashing to $400 and downgrading the stock to "Hold." The wide gap between $400 and $685 illustrates the uncertainty over whether this is a temporary hiccup or the start of a structural slowdown.

William Blair, which retained its "Outperform" call, characterized the selloff as an expectations-driven overreaction rather than evidence of fundamental deterioration. The firm points to a list of short-term pressures — ACA-related volume softness, mounting competition in China, the natural growth deceleration that comes with scale, and pricing initiatives that may amplify competitive fears — but still sees Intuitive Surgical’s earnings growth profile as industry-leading.

Intuitive Surgical at a turning point? This analysis reveals what investors need to know now.

Technically, the stock looks washed out. The 14-day relative strength index sits at 34.5, deep in oversold territory, and the share price is about 13% below its 50-day moving average of €358.27. The recent stabilization near the psychologically important €300 level suggests some dip-buying interest, but a single oversold reading is no guarantee of a trend reversal. Annualized volatility has surged above 54%, reflecting the market’s jittery assessment of the months ahead.

With the second half of the year looming, Intuitive Surgical faces a classic test: can a company that keeps beating revenue and system-shipment forecasts convince investors that its core growth engine — US procedures — is not permanently broken? The answer, for now, divides the analyst community as sharply as the price targets do.
