Wall Street had priced in a guidance upgrade, but Intuitive Surgical only delivered a confirmation — and the market responded with its harshest punishment in months. The robotics surgery pioneer lost more than 14% in a single session last Friday, tumbling to a new 52-week low of €302, as investors zeroed in on what the company didn't say about its 2026 procedure outlook.

The drubbing erased a staggering $41 billion in market value from the January all-time high of €516.50, leaving the stock 28% below its 200-day moving average and at its cheapest relative to trailing earnings in years. For a company that just posted record quarterly revenue and beat analyst estimates on both the top and bottom lines, the sell-off underscores how much the narrative has shifted from growth-at-any-price to sustainability of momentum.

The Guidance That Wasn't Enough

Intuitive Surgical reported adjusted earnings per share of $2.80 for the second quarter of 2026, comfortably ahead of consensus, on revenue of $2.89 billion — up 19% from the $2.44 billion posted a year earlier. A $28 million after-tax benefit from retroactive tariff refunds added $0.08 to the bottom line. The company also placed 468 da Vinci systems, versus 395 in the prior-year period, with 246 of those being the newest da Vinci 5 model.

Yet the headline numbers were not enough. Management reaffirmed the existing 2026 procedure-growth range of 13.5% to 15.5%, with the expected outcome near the midpoint of 14.5%. Investors had been banking on an upward revision, given the strong first-half performance and the accelerating adoption of da Vinci 5. The unchanged outlook was taken as a signal that the pace of expansion is peaking.

Should investors sell immediately? Or is it worth buying Intuitive Surgical?

"The market was pricing in a raise," wrote one analyst, who declined to be named. "When you don't deliver that, the stock gets repriced — especially at these valuation multiples."

Where the Slowdown Is Biting

The source of the caution is a clear deceleration in the US market, which remains Intuitive Surgical's largest and most profitable region. US procedure growth slowed to approximately 12% — the weakest pace since the pandemic-era lull of 2020-2021. The installed base in the US expanded just 8.7%, another multi-year low. Management pointed to two factors: the expiration of enhanced Affordable Care Act subsidies, which has caused some patients to lose insurance coverage and delay elective procedures, and the simple mathematics of a growing base from which to grow.

Outside the US, the picture is mixed. Europe and Asia posted solid procedure growth, but China remains a significant drag. Tender activity has declined, domestic robotics competitors are gaining traction, and government pricing pressures under Beijing's 15th Five-Year Plan are squeezing margins. CEO Dave Rosa flagged ongoing regulatory adjustments — including changes to billing codes and procurement quotas — as additional headwinds in the region.

Analyst Targets Slashed, but Conviction Holds

Nearly every major Wall Street house trimmed its price target after the earnings release, though none downgraded the stock outright. Leerink cut its target from $573 to $454 but kept an "Outperform" rating. Citi went from $590 to $500 and removed its 90-day catalyst-driven buy call, citing growing healthcare policy risk. JPMorgan lowered its target to $450, while Mizuho trimmed to $400 and stuck with "Hold," noting sluggish demand for accessory components. Even the Street's most bullish voice — Bernstein — reduced its target from $750 to $685, albeit while maintaining "Outperform."

Other cuts came from Raymond James ($577 to $483), RBC Capital ($600 to $575), Stifel ($670 to $550), TD Cowen, BTIG, and Baird. The resulting target range now spans $450 to $685, a wide spread that reflects deep disagreement over how much near-term caution should discount a long-term growth story that remains fundamentally intact.

Intuitive Surgical at a turning point? This analysis reveals what investors need to know now.

Technical Damage and Silver Linings

The sell-off has left clear scars on the chart. The 14-day Relative Strength Index sits at 29.9, deep in oversold territory — a level that historically has preceded short-term bounces for large-cap stocks undergoing a rapid valuation reset. The stock now trades below its 50-day ($360.34), 100-day ($382.71), and 200-day ($419.66) moving averages, and for the first time in years has breached its 200-week moving average, a support that had held through prior downturns. Annualized 30-day volatility has jumped to 54.28%, reflecting the ferocity of the repricing.

But beneath the market noise, the company's financial foundation remains rock-solid. Intuitive Surgical ended the quarter with more than $8.6 billion in cash, cash equivalents, and investments — and zero long-term debt. That balance sheet provides ample firepower to continue funding R&D and production capacity expansion without external capital, regardless of where the stock trades.

Whether the procedure-growth deceleration stabilizes in the coming quarters will determine if analysts' still-bullish price targets prove justified. If the US slowdown persists, another round of target cuts may be unavoidable. For now, the market has voted with its feet — and the message is that even a record quarter can't save a stock when expectations have run too far ahead.
