Moderna delivered the clinical proof investors had been waiting for on its personalized cancer vaccine. What the market did with that proof on Thursday was something else entirely.

The stock fell more than 25% during the session, according to Reuters, as shareholders banked profits following the run-up that preceded the release. The slide moderated to 1.1% on Friday. The trigger for the timing was mundane: results from the Phase 3 trial appeared early on Oncology Central ahead of the scheduled presentation, prompting Moderna and Merck to publish the data sooner than planned. Reuters pinned the selloff on expectations that had already been priced in rather than on any flaw in the findings.

What the INTerpath-001 Data Show

The trial enrolled 1,137 adults whose melanoma had been fully surgically removed. Patients received either intismeran autogene plus pembrolizumab — Merck's Keytruda — or a placebo alongside pembrolizumab. Adding the individualized mRNA therapy cut the relative risk of recurrence or death by 42% versus pembrolizumab alone. The risk of distant metastasis or death fell by 38%.

Absolute numbers sharpen the picture. At two years, 82.2% of patients in the combination arm remained recurrence-free, compared with 70.0% in the control group. Moderna also reported a statistically significant improvement in recurrence-free survival at a median follow-up of 24.6 months. Overall survival data are not yet mature, and monitoring continues.

Should investors sell immediately? Or is it worth buying Moderna?

Safety looked manageable. Treatment-related adverse events of Grade 3 to 5 occurred in 14.2% of the combination group and 13.4% of controls. The two companies said they are in dialogue with regulatory authorities.

The mechanism behind the drug is what separates it from a conventional shot. Intismeran is built from each patient's tumor sample and designed to train the immune system against that specific cancer after surgery. It is a treatment, not a preventive vaccine for healthy people — a distinction that carries straight through to how it must be manufactured.

A Factory Attached to Every Prescription

That production model is where the investment case gets complicated. For an individualized therapy, manufacturing is not a downstream technical step; it is part of the treatment itself. WIRED has flagged the fabrication and cost hurdles, and scaling bespoke production remains an open challenge even after positive data. A convincing trial readout does not by itself answer how broadly the therapy can be deployed in practice.

RBC analyst Trung Huynh sees approval as likely but commercial success as considerably less certain. He points to potential competition from immunotherapy administered before surgery, which could offer similar efficacy at lower cost. That comparison is contested, since the patient populations differ — making it a competitive argument rather than evidence of a superior alternative.

Regulatory timing adds another variable. Moderna is targeting FDA approval for intismeran in 2027. Until then the therapy remains experimental and available only within trials.

One Tumor Type Is Not a Platform

Moderna and Merck are also testing the approach in kidney, bladder and non-small cell lung cancer. Results in those indications are pending, and the melanoma win cannot be read as proof of efficacy across an entire product family.

On the sell side, TD Cowen had raised its price target on Moderna to $90 from $40 on Wednesday, with analyst Tyler Van Buren keeping a "Hold" rating. A sharply higher target without an upgrade in recommendation captures the tension in the stock: the science is advancing faster than the conviction about what it is worth.

Moderna's next scheduled financial update is due on November 6, 2026, when it reports third-quarter 2026 results. Between now and then, the questions that matter shift from whether the vaccine works — the data now say it does — to how a treatment made one patient at a time becomes a business that pays for itself.
